Much loved son of Mum and Dad, adopted dad Paul, brother of Daniel, Christopher, Declan and Dad to Casey.
Jordan will be sadly missed by all his family, friends and everyone who knew him.
A service to celebrate Jordan's life will be held at West Lancashire Crematorium on Tuesday 11th November at 11am.
If you're going to wear a tie, make it a red one and to wear a poppy please.
